Direct deposit form

It requests banking and taxpayer information. Do not send completed paperwork through Tenise’s website. Follow HAPGC’s current directions and verify its destination before submitting.

Lease rescission

This is a mutual agreement with release language, not a generic lease termination notice. Read every provision and get qualified legal advice if the effect is unclear.

Utility allowance schedule

The HAPGC PDF includes a schedule dated June 1, 2026, and an OMB approval date that expired April 30, 2026. Confirm HAPGC still accepts this worksheet before relying on values.

Rent increase request

The form contains an agency timing-review field. Ask HAPGC about the current notice period, submission method and effective-date rules for the specific unit.

The housing authority makes program decisions. A form submission or published rent reference does not guarantee a rent amount, approval or move-in date.
